What to Bring:
- Driving license or photo ID
- Insurance Card
- Credit card or cash for the co-pay or visit fee
- A list of your medications or the medications themselves
- Minors require the written consent of a legal guardian prior to evaluation
On arriving at the clinic please sign-in at the front desk. You will then be asked to review our Privacy Practices and complete the following forms.

Collection of Deductible/Coinsurance Form
Acknowledgment of Receipt of Privacy Practices Form
Lab Notice
You may print out these forms and bring in your completed forms if you wish to
save time on your arrival at the clinic. If you have questions about how to complete the forms please ask us when you come to the
clinic.
After completing the forms our receptionist will:
- Photocopy your insurance card
- Photocopy your driving license; and,
- Collect your urgent care co-pay
- Triage. An inquiry into the reason for your visit, review of your medical history and assessment of your vital signs (blood
pressure, heart rate etc.)
- Physician Evaluation
- Discharge Instructions. You will be provided with information noting the findings from your visit, the physician's treatment
recommendations, and follow-up instructions. You will receive a copy of these instructions to show to your primary care
physician. You will receive prescriptions for any medications prescribed upon discharge.
